We are pleased to announce the launch of our remote clinic in partnership with Suvera, a trusted digital health NHS partner. This service enables you to manage your long-term conditions from the comfort of your home, providing direct access to qualified pharmacists who will thoroughly review your health concerns, medications, and symptom control.

Transitioning to a remote clinic offers several benefits:

  • Reduced Waiting Times: Timely appointments minimize delays in receiving care.
  • Enhanced Accessibility: Easier access to clinicians when needed.
  • Comprehensive Management Plans: Prompt development and implementation of personalized care plans.

Accessing the Suvera Patient Portal

You will receive a text message inviting you to join our remote clinic, including a link to Suvera’s secure patient portal. Log in using your full name and date of birth to submit health information, such as blood pressure readings and health questionnaires. All data shared is confidential and accessible only to your GP practice.

Consultations with Qualified Pharmacists

Our remote clinic is staffed by highly trained pharmacists who will:

  • Discuss each of your conditions in detail.
  • Review your current medications.
  • Assess how well your symptoms are controlled.

These consultations are conducted via phone or video call, allowing for thorough and personalized care.

Ongoing Support

Following the review of your submitted information, our care team will determine appropriate next steps, which may include:

  • Scheduling follow-up appointments.
  • Adjusting medications.
  • Providing lifestyle advice on diet and exercise.

About Suvera

Suvera is a UK-based, Care Quality Commission (CQC) registered virtual clinic that collaborates with NHS organisations across 20 Integrated Care Board areas nationwide. Their team comprises experienced GPs, pharmacists, pharmacy technicians, and care advisors dedicated to supporting patients in managing their health.

By partnering with Suvera, we aim to enhance your healthcare experience, providing accessible, efficient, and comprehensive care for your long-term conditions.

How do the have my data?

Suvera operates as an extension of our practice, which means they can only process data on our instruction. Our contract with then requires them to keep all information confidential and secure.  This means the data shared with Suvera will never be sold or shared with anyone.

Can I still see my GP?

Absolutely!  Suvera’s service is designed to work alongside us, not replace us.  You can still see your GP whenever you need to.  Suvera provides additional support to help you manage your health more conveniently from home.
